Most of the time, whilst Scholz was speaking, Biden looked as though he had nodded off. I think they are deliberately feeding him placebos instead of his proper medication, which is why he has become more senile.
What they have been feeding him are amphetamines so he can focus when making speeches. It is a common practice with Presidents going back at least to Kennedy.
That is why they age so much in office, tweaking for 4 years puts a lot of wear on you.
